Audit Level Instances

Sweep the open level for Level Instances that point at levels which no longer exist, Packed Level Actors whose pack produced nothing, and instances that contain the level they are placed in. None of these produce an error anywhere: the symptom is a hole in the level, in a place where something used to be.
